A　诊断试剂临床质量评价要点

从临床应用角度考核检验试剂的可靠性，是以其能否区分健康与疾病的能力作为依据的。目前还很难找到100%可靠的试验，人芳香酶(ARO)检测试剂盒任何试验都会出现假阳性或假阴性。判断试验的可靠性常以其灵敏度及特异性作为考核标准。临床应用的灵敏度用疾病患者试验阳性的百分率表示，特异性以无病者试验阴性的百分率表示。

B　临床考核血清盘的制备要求

1、 采用人的原血清；

2、人芳香酶(ARO)检测试剂盒血清盘应具有相应的稳定性；

3、 血清盘中样本不含防腐剂，或只含极微量的、不影响检验结果的防腐剂；

4、 血清盘所包含的阴性样本和阳性样本约各占一半；

5、 阳性样本中，应有一定数量的强阳性和弱阳性样本；

6、 血清盘中应有一定数量的临界值上、下含量的样品，以检验试剂的灵敏度。

7、 血清盘中应包含与该项检验相关的病种样本和已积知具有干扰物质（RF因子）的样本，以检验试剂的特异性。

C　临床考核血清盘的建议

   

人芳香酶(ARO)检测试剂盒以抗-HBc-IgM为例，部临检中心收集近百例临床肝炎病人的样本，经美国abbott公司抗-HBc-IgM试剂反复检验筛选。选出血清70份，其中阳性29份，阴性41份，组成抗-HBc-IgM临床考核血清盘。在70份样本中，除7份为无病历的质控血清外，抗-HBc-IgM阳性的22份样本中含临床诊断急性肝炎16例、慢性活动性肝炎5例、重症肝炎1例；抗-HBc-IgM阴性的40份样本中，含临床诊断慢性迁延性肝炎24例、急性肝炎例（均为恢复期采的血样）、慢性活动性肝炎8例（其中5例为恢复血样）。

因此，这套抗血清盘用于商品试剂的临床考核，可以将临床上乙肝急性期、慢性活动期病人区分开，具有临床诊断意义。
